Summer Outreach 2014
In addition to Atrial Fibrillation prevention and medication safety, a health education about Fall Prevention is added in Summer Outreach 2014. As Chinese Medicine students joined the team for the first time, the team roster has expanded to 170 student volunteers and 45 alumnus registered pharmacists and nurses.
Between July to early September, the team has visited 14 elderly community centres, conducted 17 outreach sessions, and served 1010 elders in Hong Kong.
Meanwhile, the team would arrange follow-up visits according to the conditions of some elders.
Content:
1) Blood Pressure and Glucose Monitoring
2) ECG (Electrocardiography) Monitoring
3) Provide Health and Medicaiton Safety Education
4) Health Survey
